The maximum brightness of the Lumintop TD15X varies with different battery configuration:

2x CR123A or 1x 18650, Maximum 580 lumens
3x CR123A (with included extender), Maximum 720 lumens
4x CR123A or 2x 18650 (with included extender and an extra optional extender), Maxmium 720 lumens

So to reach maximum brightness of 720 lumen with rechargeable 18650 batteries, you will need this extra extender.
